- Vision Care
The purpose
of the vision care assistance program is to assist military
family members with the cost of prescription eyeglasses for
themselves and/or their children. This program does NOT include
purchasing contact lenses.
INFORMATION REQUIRED:
Signed Application on file, Military Service Verification (valid dependent ID
Card), and a copy of the CURRENT eye exam from the optometrist
QUALIFICATION:
Service member does not need to be deployed, but families of deployed service
members have priority.
Active Duty Military members do not qualify as they are provided military issued
eyewear.
